Book excerpt: The purpose of this analysis is to determine where, how, & when physiotherapy services are best delivered to optimize functional outcomes for patients after they undergo first-time total hip replacement or total knee replacement, and to determine the Ontario-specific economic impact of the best delivery strategy. It first provides background on the target population, the physiotherapy rehabilitation treatment & its use in Ontario, how the effectiveness of rehabilitation therapy is measured. It then presents results of a review of the literature to determine the effectiveness of: in-patient physiotherapy after discharge from an acute care hospital setting compared with outpatient physiotherapy in a clinic- or home-based setting; patient self-administered home exercise with or without outpatient clinic- or home-based physiotherapy services; and preoperative physiotherapy for patients scheduled for primary total knee or hip replacement surgery. The final section contains an economic analysis of a possible shift of the current rehabilitation strategy to include more home-based rehabilitation, a description of physiotherapy service providers in Ontario, and a proposed integrated model of care. Includes glossary.
